Preparation for the appointment

If you have not yet provided the information below to my office (either verbally, in person, or online), please do so. Once I have this information, it will help me prepare so that I can help you with making good health insurance decisions.

  1. If you are interested in plans that cover your prescriptions, it is helpful for us to know what medications you need to have covered. This way, we can research things like which tier levels the medications are, what copays to expect, etc. To fill out your medication form, we highly recommend that you take a few minutes and complete our intake form here - Click Here.

  2. If you are considering a Medicare Advantage plan, we’d like your provider list [we will not need a provider list if you are simply shopping for a Medigap plan - like Plan F, Plan G, Plan N, etc.]. This way we can look up your providers ahead of time to see which plans they are in-network with. For example, if your doctor takes ABC plan but not XYZ plan, then it would be helpful to know that before wasting time on XYZ plan. To complete your Provider List - Click Here. [Note: Medicare Supplement plans have no networks and allow you to go to any provider that takes Medicare. If that’s the main plan you are interested in, you can skip the provider list as we will not need it]

Enrollment Prep (when applicable) - you may need access to 3 items:

  • Medicare Card - if you have a Medicare card, it’s best to have this with you during our appointment in case we need to put it on an application for Medicare-related health insurance. If you can’t find it, or forget it, it’s OK we can often locate it, but it is easiest if you have the red/white/blue Medicare card available.

  • Bank Info for Premium payments - If you are coming in to enroll in an insurance policy, it’s best (though not always required) to bring your bank info with you. Either a copy of a check or a smartphone with access to your bank’s routing number and account number.

  • E-mail access (smartphone with e-mail) - Also, some signatures now require access to your e-mail, so again, a smartphone with e-mail access is usually helpful, though not always required.
